Intermenstrual bleeding (ovulation bleeding) refers to the phenomenon of vaginal bleeding during the ovulation period of the menstrual cycle. This type of bleeding is usually light, not as abundant as normal menstrual bleeding, and usually only lasts for a few hours to a few days.

Causes of ovulation bleeding

  • Mucosal shedding: During ovulation, the ovary releases a mature egg, and the lack of sufficient progesterone levels can lead to shedding of the ovarian surface mucosa, causing slight bleeding.
  • Blood vessel dilation: The ovulation process may cause the ovarian blood vessels to dilate, which could lead to minor damage to the blood vessel walls, resulting in a small amount of bleeding. 

Symptoms of ovulation bleeding

  • Vaginal bleeding: Ovulation bleeding is usually manifested as light vaginal bleeding, often brownish, pink or light red in color.
  • Abdominal discomfort
  • Lower back pain

TCM perspective of ovulation bleeding

From the perspective of traditional Chinese medicine, ovulation bleeding is related to liver stagnation, blood heat, and blood stasis.

Imbalances in the liver, spleen and kidney functions can also affect the endocrine system, thereby causing ovulation bleeding.

TCM Treatment of ovulation bleeding

Ovulation bleeding usually does not require special treatment, but if the bleeding is excessive or prolonged, it can be stabilized through traditional Chinese medicine and acupuncture to regulate the endocrine system.

Herbal medicine: The Chinese medicine practitioner will prescribe appropriate herbal formulas based on the patient’s specific symptoms and constitution, to regulate the functions of the liver, spleen and kidney, and thereby adjust the hormonal balance.

A “cyclical therapy” approach may be used, providing different prescriptions during the different phases of the menstrual cycle (post-menstrual, pre-ovulation, post-ovulation, pre-menstrual) to stabilize the hormones and treat ovulation bleeding.

Acupuncture: Acupuncture can regulate the endocrine system, and also promote pelvic blood circulation.

Specific acupuncture points related to gynecology, such as the uterus point, Zusanli, and Yinlingquan, can be needled to stop bleeding and regulate hormones to prevent future occurrences of ovulation bleeding.

  • Observe ovulation: Understanding one’s menstrual cycle and tracking ovulation days can help predict potential ovulation bleeding.
  • Relax the mind: Stress and anxiety may adversely affect the menstrual cycle, so maintaining a good mental state and relaxation can help prevent ovulation bleeding.
  • Maintain a healthy lifestyle: Maintaining a balanced diet, moderate exercise, and adequate rest can help maintain overall physical health, which may also contribute to the stability of the menstrual cycle.
