Rosacea, commonly known as “alcoholic nose”, is a chronic inflammatory skin disease.

This condition is more common in women, with the main features being chronic erythema and papules on the face, accompanied by inflammation and a feeling of pain.

In men, the type characterized by fibrovascular hyperplasia of the nasal glands (alcoholic nose) is more common.

Causes of rosacea

  • Vascular dilation: Facial vascular dilation and flushing are one of the main characteristics of rosacea. This may be related to abnormal vascular wall response and endothelial cell dysfunction.
  • Demodex mite infection: Some studies suggest that Demodex mite infection may be associated with rosacea. These tiny mites may trigger an immune response, leading to skin inflammation and erythema.
  • Endocrine imbalance: Endocrine imbalance is also considered an important factor in rosacea. For example, hormonal changes during menopause in women may lead to vascular dilation and increased skin sensitivity. 

Symptoms of rosacea

  • Chronic facial erythema: Erythema usually appears on the cheeks, nose, forehead, and chin, and may spread and worsen.
  • Skin sensitivity: Patients may be sensitive to irritants such as cosmetics, cleansers, and climate changes, with the skin experiencing a burning, stinging, and itching sensation.
  • Papules and pustules: Rosacea patients may develop red papules, pustules, and comedones, which can be painful and uncomfortable.

Three main TCM subtypes of rosacea

  • Blood-heat type: The skin appears red, swollen, and burning, with obvious inflammatory reactions and the formation of papules and pustules.
  • Qi stagnation type: The skin shows papules and nodules, with a dark purple skin color and tenderness.
  • Liver-spleen disharmony type: Mainly manifested as comedones and papules, with oily skin, often accompanied by symptoms such as epigastric fullness and abdominal distension, which tend to worsen before menstruation. 

 Three-step TCM treatment of rosacea

Herbal medicine: The practitioner will prescribe herbal medicines based on the patient’s symptoms and tongue and pulse analysis, using methods such to regulate energy balance and calm the skin.

Topical ointment: The practitioner will also provide local application of Chinese herbal ointments, washes, etc. to reduce inflammation, redness, and itching.

Acupuncture: Acupuncture can regulate hormonal levels and promote blood circulation, alleviating symptoms and promoting skin repair.
